Create your Form

 
 

NOTE: If you are requesting any Patient Identifiable Information (PII) in your Forms we would advise contacting your local Information Governance team before doing so. Please make sure your Form Data email address is correct and that all Forms have been fully tested before publishing on the Live site.

 

Add your Form to a page

 

  • Go to the page where you'd like to add the form 
  • Select the Pencil Icon and Inline Edit 
  • Drag and Drop the Form Plugin
  • In the Form Settings find and select the name of the form which you would like to display
  • Label & Position Tabs: Set and position a title, select a width and align (If required)
  • Publish the page to save your changes

You can only use one form on a page at a time
